Abstract
To avoid preventable readmissions, Chronic Obstructive Pulmonary Disease (COPD) is included in Medicare’s Hospital Readmission Reduction Program (HRRP). COPD exacerbations are the most common cause of readmissions for patients with COPD. Utilizing resources such as home health services could possibly help prevent readmissions. Several studies have used telemedicine as well as experienced pulmonary nurses to monitor for compliance and provide additional instructions. However, the results of the studies that have been conducted provide conflicting results. The majority indicated some benefit in decreasing readmission rates as well as increasing patient satisfaction. Other resources may prove to be more beneficial, in addition to home health services, such as studies utilizing case management, physician home visits, and hospital-at-home models. 19
