Odontoid fractures can occur throughout all age groups as a result of a high energy trauma, or hyperextension of the neck. However, there are patient’s that carry an increased risk for atraumatic odontoid fractures if they carry the diagnosis of rheumatoid arthritis (RA). This case report will discuss the presentation of a patient with a history of RA that presents with atraumatic neck pain, how to appropriately evaluate the patient, odontoid classifications, and treatment.
