Abstract
ABSTRACT
Purpose: The purpose of this article is to review the role of population health management in the value-based health care system with an appreciation to its current and potential roles within the national health care systems of the United States of America, the United Kingdom, and the
European Union.
Method: A PubMed literature search was conducted with the following search terms: population health management, value-based care, public health, data analytics, patient engagement, delivery of health care, and patient care management. Thirteen pertinent articles were retrieved and served as the basis for this clinical review. Filtered by year and subject, exclusion criteria included articles older than five years and those concerned with either fee-for-service or nonvalue-based health care management.
Results: A literature review resulted in a plethora of evidence-based research to demonstrate the positive outcomes on patient engagement and community wellbeing when population health management tools are implemented into a value-based health care system.
Conclusion: Alongside growing recognition that social determinants of health influence individual and community wellbeing, a robust population health management program can result in improved outcomes for both patient and practice, while producing a leaner, more comprehensive, and financially efficient health care system. Consequently, population health management teams and tools have the capacity to optimize the wealth of patient information and leverage the Quadruple Aim through dedicated coordination of patient care, team collaboration, and optimized technology.
