Abstract
Medication reconciliation is more than a mandated portion of the medical record. When updated, the medication list is a valuable tool that benefits the patient by reducing medication interactions. Correct medication reconciliation is an essential tool for providers to utilize in developing a plan of care. This article aims to highlight the problems of adequate medical reconciliation in urgent care and E.D.s. A review of currently published sources showed several options none are shown to be fully effective. Easy solutions are not present to improve this system.
