This article aims to review the approach to the diagnosis, monitoring, and management of Hepatitis D infection and increase awareness of this complicated disease. Hepatitis D is highly virulent, yet often underdiagnosed, leading to increased risk for fibrosis, cirrhosis, carcinoma, and mortality. With evolving therapies, controlling the hepatitis D virus before its progression to severe disease may be possible. Hepatitis D infection can be identified with multiple diagnostic testing methods, with less invasive scoring tests being the most recent advancement. In addition to serologic testing, other less invasive scoring methods aid in monitoring the progression of the disease. Recent advancements in treatment options have fewer side effects and better efficacy, allowing for better virus control, reducing the risks for disease progression, and improving overall treatment outcomes. Despite treatment advances and disease resolution, continuous monitoring is still necessary, as disease recurrence is possible. With an increased understanding of the relationship between hepatitis B and hepatitis D infection, in addition to available testing and treatment options, hepatitis D may lessen the burden on the healthcare system.
