Abstract
This review evaluates how integration of community specialty practices into large health systems influences clinical and operational outcomes. Consolidation continues to change how care is delivered in the United States. The impact of specialty practice acquisition on quality, access, cost, and workforce performance remains inconsistent. A literature search was conducted in PubMed, including peer-reviewed articles published between 2014 and 2025. Search terms included specialty practice acquisition, physician consolidation, vertical integration, and workforce culture. More than 30 peer-reviewed studies were included to evaluate post-acquisition trends. The review focused on key factors influencing these outcomes, including workforce structure and care delivery models within integrated systems. Findings show mixed results following integration. Some studies report improved financial stability, while others show increased healthcare spending without consistent improvement in quality. Consolidation is also associated with reduced market competition. Variation in outcomes appears to reflect differences in implementation, including care-delivery processes, workforce structures, and operational design. Integration alone does not create value. Success depends on clear role definition, standardized workflows, alignment of care models, and intentional integration across the system.
