Abstract
ABSTRACT
Social determinants of health (SDoH), the non-medical conditions in which people grow, live, and work, are more than just background factors; they are the primary drivers of chronic disease and health disparities. These factors, including economic stability, access to food, safe housing, and reliable transportation, provide patients with the ability to manage chronic conditions. Despite the known impact of SDoH, a critical gap remains in clinical practice due to a lack of standardized screening tools and provider awareness. This study assesses systematic approaches to neutralizing these barriers and improving care coordination. Our findings demonstrate that identifying and addressing these barriers leads to measurable improvements in clinical outcomes and patient engagement. To achieve sustainable population health and true health equity, healthcare systems must move beyond traditional medical models and formally integrate SDoH-informed strategies into routine care delivery.
