Abstract

ABSTRACT

Hypertension is one of the most prevalent and modifiable risk factors for cardiovascular morbidity and mortality in the United States. African American patients experience disproportionately higher prevalence and poorer control of hypertension compared to other racial and ethnic groups, contributing to excess cardiovascular burden. These disparities reflect the influence of social determinants of health and highlight the need for culturally responsive, community-based interventions.

This doctoral project evaluated the effectiveness of a culturally tailored community intervention to improve hypertension control among African American patients through education, lifestyle modification, self-monitoring, and structured collaboration with local healthcare centers. The intervention engaged community healthcare workers to provide counseling, improve adherence, and empower participants to better manage their condition. Findings indicate that such interventions enhance patient knowledge, promote adherence to recommended behaviors, and improve blood pressure outcomes. Evidence from comparable community programs shows reductions in systolic blood pressure of up to 21 mmHg and improvements in medication adherence exceeding 10%.

Expanding these initiatives across multiple sites, integrating digital health tools, and ensuring long-term sustainability through supportive health policies could further mitigate disparities in hypertension outcomes. Improving hypertension control among African American communities is both achievable and essential for advancing health equity and reducing cardiovascular morbidity and mortality nationwide.
