Abstract

The objective of this review is to synthesize the clinical outcomes of mobile health (mHealth) interventions in managing hypertension and type 2 diabetes within low- and middle-income countries (LMICs). This research is critical as non-communicable diseases (NCDs) account for 77% of all deaths in these regions, frequently striking younger populations and imposing severe economic burdens. A literature search identified 30 original peer-reviewed articles published between 2012 and 2025, including randomized controlled trials, quasi-experimental studies, and prospective cohort studies (≥ 50 participants each). The findings demonstrated significant clinical and behavioral improvements across the reviewed populations. Specifically, integrated app-based trials yielded systolic blood pressure (SBP) reductions of approximately 10 mmHg—compared to just 3 mmHg in earlier SMS-based interventions—alongside statistically significant decreases in HbA1c levels. Digital reminders successfully mitigated forgetfulness, shifting patient agency and improving adherence to medication and dietary regimens. Crucially, these interventions reduce the "transportation tax" and logistical barriers faced by rural patients, facilitating decentralized care without requiring a proportional increase in the healthcare workforce. This study suggests that mHealth acts as a catalyst for patient self-management and a force multiplier for clinicians, though it should function as an adjunct to—rather than a replacement for—traditional in-person care. In conclusion, digital health is an effective tool for optimizing outcomes and establishing sustainable health habits in resource-constrained environments. Future research should prioritize the long-term longitudinal efficacy and systemic sustainability of these interventions beyond short-term trial periods.
