Abstract

ABSTRACT

We present the case of a 58-year-old woman with recurrent metastatic radiation-induced angiosarcoma of the breast (RAASB). RAASB is a rare but highly aggressive malignancy that occurs as a late complication of breast radiotherapy. Management of metastatic disease remains challenging due to limited systemic treatment options and a paucity of evidence guiding second‑line therapy. Although surgical excision remains the standard of care for localized disease; systemic therapy is unavoidable in a metastatic setting. Current National Comprehensive Cancer Network (NCCN) guidelines recommend systemic agents such as docetaxel, paclitaxel, and vinorelbine for angiosarcoma, while targeted therapies including pazopanib have demonstrated encouraging activity. Emerging evidence also suggests a potential role for immunotherapy. This case report aims to describe the clinical course and treatment response of a patient with recurrent metastatic RAASB treated with Gemcitabine–Docetaxel–Bevacizumab (GDB) regimen and to review available alternative systemic treatment strategies. She had earlier undergone neoadjuvant chemotherapy, surgery, and adjuvant taxane-based chemotherapy. She was subsequently treated with the GDB regime, resulting in a significant clinical and metabolic response. The patient exhibited a progression-free survival of ten months, which is still ongoing as of the reporting date, and is currently being treated with maintenance therapy (Gemcitabine and Bevacizumab). This case emphasizes the possible effectiveness of the Gemcitabine–Docetaxel–Bevacizumab combination as a salvage therapy for metastatic RAASB and highlights the necessity for additional investigation to determine optimal systemic treatment approaches for this uncommon and aggressive condition.
